My First Visit

Our staff looks forward to meeting you and discussing your goals. We strive to keep our appointments on time as scheduled. Prior to your appointment, we encourage you to fill out the patient forms that are located on our website so you can accelerate your check-in process. Please download, print, and complete the forms and bring them with you to your appointment. If you are unable to download our patient forms, then it is recommended that you arrive at your appointment 15 – 30 minutes early so you can have ample time to fill out the appropriate forms.

At our dental practice, we want to assist you in obtaining the preventive, cosmetic, and restorative dentistry services you need for your dream smile! Our team’s primary mission is to deliver the finest and most comprehensive dental services available today. Our office works with you to streamline insurance processing and account handling to be as affordable as possible. We accept cash, credit cards, and checks. For your convenience, we also offer GreenSky as a financing option.

Insurance Options:

We are currently out of network with insurance providers, but we are happy to file a claim for you to determine if your plan has any out-of-network benefits. Please give our office a call to determine the best option for you.
